Once you have downloaded the Spruce app and made an account, then you can initiate telemedicine visits such as video visits.

Please note you will need to download the documents below that need to be signed and an ABN that will need to be signed if you are a Medicare patient. Please sign those forms and return to us via email or fax.

Please note that while Spruce is HIPAA complaint, email is not HIPAA compliant.

You can fax the signed forms back to our office at 813-641-0009 which is a HIPAA compliant way to return the forms to us.

These forms will need to be completed prior to the Telemedicine list.

Self pay payment will be collected prior to your telemedicine visit. For those utlizing insurance, your claim will be filed prior to your telemedicine visit. Our staff will advise you of that based on your insurance plan or if you are a self pay patient.
